2004年歐洲國家盃淘汰賽於2004年6月24日至2004年7月4日之間舉行，所有比賽時間將以歐洲西部夏令時間（UTC+1）為基準。共有8支球隊從4組小組賽晉級到本屆淘汰賽階段，此階段比賽以單敗淘汰制執行，贏得比賽的隊伍將進到下一輪階段，最終在第三輪決賽期間決定冠軍隊伍。本屆決賽將於2004年7月4日在里斯本盧斯球場舉行，最終希臘以1-0險勝東道主葡萄牙，首次奪得歐洲國家盃冠軍寶座。
形式
淘汰賽階段比賽常规时间為90分鐘，若常规时间內（包含90分鐘及补时）仍为平局，則進入30分鐘的加時賽。若加时赛之后仍然平局，將以互射十二碼決定下一輪球隊，共進行5輪點球，若在5輪內仍平局則將增加輪次繼續比賽，直到分出勝負為止。本屆比賽沿襲自1984年歐洲國家盃的傳統，不舉行。
