2018年國際足協世界盃外圍賽 – 亞洲區第三圈的参赛球队由八支上一圈比赛各组的首名，及四支成績最佳的次名球隊，共十二隊參賽，因此也被称为2018年國際足協世界盃外圍賽亞洲區12 强赛。比赛日期目前定于2016年9月1日至2017年9月5日。比赛分为两大组，每组各6支球队，採用主客场双循环制。每组的前两名球队将直接获得2018年國際足協世界盃的参赛资格，而两个排名小组第三的球队将进行第四圈角逐第五名，這些隊伍同時具有2019年亞洲盃足球賽的資格。

種子排序依以2016年4月的國際足總世界排名作為依據。以下的排序按照2016年4月的世界排名，而抽籤結果剛好省掉因為沙烏地阿拉伯及伊朗關係惡化所帶來的問題。（同一年的亞冠聯賽，沙烏地阿拉伯及伊朗俱樂部之間的賽事於中立場地舉行。）
